If you are signing in for the first time, you'll need to complete all required fields. These include the CDN Contact ID number, provider's birth date, email address, tax ID, phone number, security questions, security image and password.

The CDN Contact ID number can be found on your Connection Dental Network Correspondence. If unable to locate, please contact Provider Relations team at 800-505-8880 option 4. If you have any additional questions, you can also click the Contact Us link at the top of the page, then select our Contact Form to send us a message. CDN Contact ID must be either 10 numeric or 11 alphanumeric characters with no spaces or dashes.
