Important COB Information for Participating Dentists
With Federal Employee Patients

We want to make sure you are receiving the maximum benefits for GEHA's Connection Dental Federal Plan members.

If your patient is a federal employee, spouse or annuitant and has both a federal health insurance plan and GEHA's Connection Dental Federal Plan:

  • The federal health insurance plan carrier is the primary carrier. Add the Member's health plan Member ID number to Box 15 of the ADA Form under the Primary Subscriber Information section of the form.
  • GEHA's Connection Dental Federal Plan is the secondary carrier. Add the Member's dental plan insurance Member ID number to Box 8 of the ADA Form under Other Coverage.
  • Always include both the health plan Member ID number and GEHA's Connection Dental Federal Member ID number on your claims when you submit them to each carrier.

For example, if your patient has Blue Cross and Blue Shield health insurance and Connection Dental Federal dental insurance, make sure you include both the Blue Cross and Blue Shield Member ID number and the Connection Dental Federal Member ID number on any claims you submit.

In addition, if you send us a claim with both of the Member ID numbers and you include the federal health Plan/Group Number (Box 16 of the ADA Form) on the claim, we will forward the claim to the federal health plan for you.
