Our Credentialing Department includes five highly trained Credentialing Specialists who review the qualifications of dentists to ensure that we obtain and maintain a high quality network of Participating Dentists.

Prospective CONNECTION Dental Network applications are reviewed and verified by our team of Credentialing Specialists. Dentists are initially reviewed and then re-reviewed every three years. Copies of the following information are required as part of the credentialing process:

• State license
• Federal or state DEA certificate
• Specialty board certificate
• Current professional liability insurance face sheet

Our Peer Review Committee reviews the information collected for first-time applicants, and is responsible for approving new dentists on a monthly basis.

We understand how busy our Participating Dentists' offices can be. Therefore, we have made the recredentialing process as quick and easy as possible. When the time comes for a Participating Dentist to be recredentialed, our Credentialing Department will send you a recredentialing packet consisting of a letter, a dental profile and a recredentialing application. Copies of the following information are required to complete the recredentialing process:

• Update state license
• Updated federal or state DEA certificate
• Specialty board certificate if old certificate is expired or you have become board certified since your last credentialing instance
• Updated professional liability insurance face sheet
